Transportation Waiver of Liability and Harmless Agreement
Trip: 8th/9th Grade Travel Basketball Tournament
Date: June 13, 2026 & June 14, 2026 (This is NOT an overnight trip)
Location: RDV Sportsplex Pediatrics- 8701 Maitland Summit Blvd, Orlando, FL 32810
Time: Coach Louis will communicate the times once the tournament organization releases this information
Fee: $20 per day
This is a legally binding Consent Form and Release of Liability made voluntarily by me, the undersigned adult, on my own behalf, and on the behalf of my heirs, executors, administrators, and legal representatives. The undersigned hereby acknowledges that participation in the above-named activity and travel involves inherent risk of physical injury and assumes all such risks. The undersigned hereby agrees that for the sole consideration of Synergy School of Tomorrow(SSOT) and On & Off the Court Sports Academy(OOC) allowing the undersigned to participate in Camp Field Trips and to be transported by bus/van for which or in connection with which Synergy School of Tomorrow and On & Off the Court Sports Academy(OOC) has made available any equipment, facilities, grounds or personnel for such programs or activities, the undersigned does hereby release and forever discharges Synergy School of Tomorrow and On & Off the Court Sports Academy(OOC), its members individually, and its officers, agents and employees of any and all claims, demands, rights and causes of action of whatever kind of nature, arising from and by reason of any and all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to property, and the consequence thereof, resulting from participation in or in any way connected with the above named activity. In an emergency, I acknowledge that I am solely responsible for all medical and other costs arising out of bodily injury or any loss sustained through participation in this activity. I authorize program staff to secure any licensed hospital, physician and/or medical personnel any treatment deemed necessary for the participant’s immediate care.
By signing this agreement, I accept and assume full responsibility for any and all injuries, damages, and losses of any type that may occur during transportation or any transportation-related activity. I hereby fully and forever release and discharge On and Off the Court Sports Academy, Synergy School of Tomorrow (SSOT), and their officers, owners, employees, coaches, staff, agents, insurers, and representatives from any and all claims, demands, damages, or rights of action, whether present or future, known or unknown, anticipated or unanticipated, resulting from or arising out of participation in this activity.
I have read and understand this Transportation Release and Waiver of Liability. I understand that this waiver shall remain effective for the date, activity, or transportation period listed above.
